Dementia Care Finder: Perth

Navigating aged care services can be complicated, that’s why Dementia Australia’s care finder service can help connect you to the support you need. This free service supports older people who need assistance to navigate the aged care system and do not have family members or friends who can help.

Located in the Perth Metro region (including Peel), the care finder program is suitable for Australians 65 years or older (50 years for Aboriginal or Torres Strait Islander people) who:

  • Require other government support such as health, housing services, and social supports, and
  • Are experiencing isolation, language or communication barriers and/or cognitive issues
  • Have no family, friends, a carer or a representative they are comfortable receiving help from.

This service has been made possible through funding provided by the Australian Government Department of Health and Aged Care under the Primary Health Network Program.
